Long-Term Care Ombudsman (Full-Time)
Join a nationally accredited senior center and make a meaningful difference in the lives of older adults across Philadelphia. We are seeking a compassionate, motivated professional to serve as a Long-Term Care Ombudsman, advocating for residents in skilled nursing facilities, personal care homes, domiciliary care homes, and older adult daily living centers.

- Visit assigned long-term care facilities to build relationships with residents, families, and facility staff.
- Serve as a trusted advocate and liaison, helping residents to understand their rights and access available community resources.
- Investigate and resolve complaints related to resident care, safety, rights, or service quality.
- Collaborate with residents, families, and facility staff to ensure concerns are addressed and resolved effectively.
- Maintain accurate, confidential documentation of complaints, actions taken, and outcomes.
- Support resident councils and family groups to help empower residents and encourage active engagement in their care.
- Represent resident interests during meetings with facilities and state agencies.
- Support community outreach efforts to recruit volunteers and educate community members about the Ombudsman program.
- Spend one day per week staffing the Ombudsman Intake Line, fielding calls and compiling the preliminary information needed to address a resident’s concerns.
- Perform other related duties as needed to support the program’s mission.
To be successful in this role, you will need:
- A Bachelor’s degree (required)
- A valid PA driver’s license and daily access to a personal vehicle (mileage reimbursement provided)
- Strong communication, problem-solving, and interpersonal skills
- A commitment to fairness, advocacy, and resident empowerment
- The ability to maintain confidentiality and adhere to program regulations
- Motivation and organization to thrive while mostly working remotely
This is a primarily field-based role, with staff working remotely and regularly conducting in-person visits to their assigned roster of long-term care facilities. Each Long-Term Care Ombudsman is scheduled to spend one day per week in-person at Center in the Park (5818 Germantown Ave) to manage the Ombudsman Intake Line.

Health Promotion Engagement Coordinator
The Health Promotion Engagement Coordinator at Center in the Park supports the Center’s Strategic Plan by helping to bring health and wellness programs to life for older adults, both at the Center and throughout the community. This role emphasizes proactive outreach and engagement, taking wellness initiatives on the road to connect with residents, partner organizations, and community spaces. The coordinator helps increase awareness, participation, and access to CIP programs while advancing the center’s commitment to innovation, collaboration, and inclusion.

- Serve as a passionate ambassador of Center in the Park’s mission and philosophy, clearly articulating and modeling its values through daily work and community engagement.
- Support the coordination and growth of program infrastructure, research initiatives, and training activities tied to academic–community partnerships and funded projects.
- Bring fresh, creative energy to the delivery of health promotion and wellness programs, including the Self-Management Series and other innovative, evidence-based approaches.
- Take health promotion beyond the walls of the Center—bringing programs “on the road” through community outreach that includes: Evidence-based self-management courses; Blood pressure screenings; Exercise and movement workshops; Health education pop-ups and special initiatives
- Actively build excitement and awareness around CIP’s offerings by connecting with community partners, host sites, and residents both on-site and throughout the community.
- Play a key role in expanding outreach goals and increasing participation in Center in the Park programs by meeting people where they are and creating welcoming, engaging entry points.
- Recruit participants and host sites to strengthen and grow the Center’s health promotion programs and community reach.
- Collaborate closely with the Health Promotion Supervisor and Center Services team to bring health programs, workshops, and special events—such as health fairs—to life.
- Explore, evaluate, and help implement new evidence-based health promotion programming aligned with community needs.
- Work alongside volunteers to support seasonal and national initiatives such as National Memory Screening Day and American Heart Association awareness events.
- Coordinate and administer evaluation surveys to measure program impact and continuously improve offerings.
- Represent Center in the Park at community events, actively engaging residents, partners, and stakeholders by staffing outreach tables, sharing program information, and creating welcoming connections that increase awareness and participation in Center offerings.
- Maintain accurate program data, documentation, and reports, including funder reporting for organizations such as the Philadelphia Corporation for Aging (PCA) and the Self-Management Resource Center.
- Facilitate workshops that promote community participation, overall health, cardiovascular wellness, and nutrition, including coordinating educational sessions and supporting peer educators.
- Support the Health Promotion department in the planning, coordination, and implementation of health screenings and vaccination events, including their promotion at Center in the Park
- Bring enthusiasm, collaboration, and a team-first mindset to all work, contributing positively to a dynamic, mission-driven environment.
- Participate in required meetings and trainings, actively applying new skills and knowledge to strengthen program delivery and community impact.
Knowledge & Experience Required
- Bachelor’s degree preferred, or a combination of relevant health-related education and professional experience.
- Basic understanding of the physical and emotional needs of older adults.
- Knowledge of computer systems and applications used in senior centers.
- Knowledge of program development and training.
- Basic understanding of marketing and outreach
Skills Required
- Good computer skills, including database management
- Outstanding communication skills, oral and written.
- Strong organizational, developmental, and planning skills.
- Good interpersonal skills.
- Ability to work independently & as a member of a multi-resource/task team.
This is a full-time position (35 hours per week), with the majority of time spent in the office at Center in the Park and approximately 30% of time spent in the field. Reliable transportation and access to a car are required.

Center Counselor
Are you passionate about making a difference in the lives of older adults? Center in the Park is looking for a compassionate, motivated Center Counselor to join our team! As Center Counselor, you will be the welcoming face for new senior center participants, guiding them through their first steps at the center. You will help connect older adults with the resources, benefits, and support that they need — whether it’s assisting with applications, advocating on their behalf, or simply offering a listening ear. You will be a trusted advocate who empowers older adults to age with dignity, purpose, and joy.

- Welcome & Support New Members: Complete intakes and orient new participants to Center in the Park’s programs and services.
- Connect People to Resources: Help members access benefits, entitlements, and supportive services by completing applications and providing follow-up.
- Be a Trusted Counselor: Offer support to members and their families, helping them navigate challenges with care and compassion.
- Promote Wellness: Manage CIP’s wellness screenings, providing follow-up and keeping accurate records.
- Lend a Helping Hand: Oversee our small emergency food pantry and monthly boxed food program to ensure members have access to essential items when needed.
- Coordinate Community-Based Services: Provide support and referrals to homebound older adults, including the provision of emergency assistance funding for older adults facing critical needs.
What You Bring:
- A bachelor’s degree in Social Work, Human Services, or a related field.
- Experience working with older adults, especially in social services.
- Strong communication and interpersonal skills.
- Knowledge of casework process and practice.
- Comfort with technology, including Excel and Outlook.
- A valid driver’s license and access to a vehicle (preferred).
