Our Service Coordinators serve residents of two senior housing complexes, offering care planning, linkage to resources, and coordination of resident activities.
Service Coordination
Center in the Park provides service coordination at 3 senior housing buildings: Maple Village (22 W. Rittenhouse St., Philadelphia PA 19144), Golden Age Living Accommodations (2022 Haines St., Philadelphia PA 19138), and Walnut Park Plaza (6250 Walnut Street, Philadelphia, PA 19139. All 3 buildings are affordable housing communities for adults age 62 and over.
Our service coordinators support residents with accessing resources, applying for benefit programs, and connecting with community services. We also coordinate on-site programming for residents. Maple Village residents can meet with their Service Coordinator on Mondays and Thursdays. GALA residents can meet with their Service Coordinator on Tuesdays and Wednesdays. Walnut Park Plaza’s service coordinators are available 5 days per week.

Please note: We do not manage the application process for individuals interested in moving into these buildings.
